Provides individual and group rehabilitation services in the areas of personal life, leisure, education and work.

The community mental health teams provide individual and group rehabilitation services in the areas of personal life, leisure, education and work. Their services involve the client and, if appropriate, their significant other and family members. Services include:

Individualized mental health screening and assessment
Rehabilitation goal-setting
Basic support (access to activities)
Rehabilitation readiness assessment (exploration of service options, building confidence)
Rehabilitation processes (assistance with goal achievement)
Referral to other services and advocacy

This program serves clients 19+ and can be accessed with a referral through the Access and Assessment Centre (AAC), or by a family physician, psychiatrist or nurse practitioner.
