Our clinic supports people with complex diabetes management issues through a range of services including: education and training on advanced carbohydrate counting, intensive insulin therapy and continuous glucose monitoring as well as endocrinology referral.

There are various programs offered at the centre to help educate and support people living with diabetes. These include:
  • Basic Diabetes Education: This program provides education and resources to help patients self-manage their diabetes. It provides a good opportunity for people to learn about diabetes. Suggested for anyone with diabetes.
  • Pre-Diabetes: This program provides education and resources to help people prevent or delay the development of type 2 diabetes. Suggested for anyone at risk for developing type 2 diabetes.
  • Gestational Diabetes: Learn how to self-manage with individualized support from the diabetes team. Suggested for pregnant women who have been diagnosed with gestational diabetes.
  • Carbohydrate Counting: The class offers lessons in carbohydrate counting and how to best manage blood sugars. Suggested for people using mealtime insulin.
  • Insulin Pump Follow-Up: Learn to make the most of using an insulin pump and keep your diabetes well controlled. Suggested for insulin pump users looking for guidance and support.
  • Self-Management Workshop: A 6-week intensive workshop designed to help individuals improve their health and selfmanage their diabetes care, one step at a time.
    Stemming from these are follow-up programs, which are offered on an individual basis in order to provide continued care.

    Eligible for individuals with complex management issues, including insulin therapy and presence of chronic or acute diabetic complications. Please see back of referral form for complete admission criteria.

    We do not accept referrals for:
  • Pre-diabetes
  • A1c <8.6% while on less than or equal to 2 antihyperglycemic agents that do not include insulin, sulfonylureas, meglitinides
