Seniors' Community Connector - Social Prescribing Program - Grand Forks

The Social Prescribing Program supports seniors (60+) to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
Social Prescribing is an approach to offering non-medical support that enables older adults to identify, set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local programs and services and provides regular check-ins to ensure clients are on track. Boundary Family Services has a Community Connector under the Social Prescribing program supporting older adults in Grand Forks.

Eligibility: Any older adult 60+ residing in Grand Forks who is either experiencing or is at risk of:
  • Frailty
  • Social Isolation and/or Loneliness
  • Barriers to Accessing Community-Based Resources
  • Frequent Trips to Emergency Department or Primary Care
  • Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of-Life, or Palliative Care

Accepts referrals from health care professionals, family/friends, community organizations, as well as self-referrals.
