Condition Specific Support and Services for Seniors

- Loneliness or social isolation
- Recent loss or multiple losses
- Mobility or sensory challenges
- Increased disengagement
- Coping with transitions and multiple decisions
- Difficulty finding or accessing information or resources
- Perceived need for the program
- Practical Support: Specially trained volunteer navigators help clients access resources and services in their community, while providing companionship and emotional support.
- Emotional Support: Specially trained volunteers are matched one-to-one with clients to provide consistent, caring support and to encourage the development of a close connection.
- Social Support: Nav-CARE volunteers can help clients to re-engage in their community and the activities they love.
Nav-CARE Dementia Program: Supports individuals living with dementia and their caregivers in the Greater Trail area, including Rossland, Warfield, Genelle, and the Beaver Valley. The program includes dementia-specific navigation training for volunteers to support individuals living with dementia and their caregivers. It also promotes initiatives that foster compassion and inclusion and aim to reduce stigma associated with dementia.
