Services include:
- Hormone initiation and stabilization
- Surgical care planning and referrals
- Pre-operative information and education
- Post-operative nursing care
- Limited counselling
- Hormone injection supplies
- Assistance with identity document changes
Clients can self-refer by by calling our TSC intake clerk at (604) 331-8901 or through our online referral form. The client's primary care clinician can also fax a referral (see fax number below). The intake clerk will contact you to follow up and confirm your waitlist placement within 1-2 business days. Please let your physician know about the best way for us to contact you.
Eligibility: Services are provided to people 15 and older living in the Vancouver Coastal Health region. Youth (aged 15-24), refugee claimants, and individuals with post-operative complications who live outside the VCH Region will be given special consideration on a case-by-case basis. Clients who live outside of the VCH region can contact Transcare BC’s Care Coordination team for help with finding care in their area.
