Adult Tertiary Mental Health Rehabilitation - For people with serious mental illness

Provides 24-hour specialized in-depth wellness and recovery-based rehabilitation services for adults (19+) with serious mental illness.
The treatment team has specialized training in psychosocial rehabilitation to provide 24-hour specialized in-depth wellness and recovery-based rehabilitation services for people with serious mental illness. The goal is to help clients gain the skills and abilities they need to increase their independence and quality of life, returning to their home community with supports in place. Clients can expect to receive the support required to work toward their wellness goals. These are developed individually in collaboration with their care team.

Clients' length of stay ranges from 6-24 months; some people may benefit from a longer stay.

​This service requires a referral by a case manager, community mental health team or a psychiatrist in hospital. Referrals are accepted through the Centralized Access & Discharge Line.

Eligibility: ​The program is for adults 19 or older with severe and persistent mental illness and does not have an acute medical condition.

​A referral is required.
